Who are we?
We are North Middlesex University Hospital NHS Trust, also known as North Mid.
What do we do?
We provide hospital and community healthcare services in Enfield and Haringey and surrounding areas including Barnet and Waltham Forest. This includes overnight stays in hospital, outpatient clinics and tests such as X-rays, and community health care near where you live. To do this we sometimes need some of your personal information.
What is personal information?
Personal information is information about you. It might be your name, your date of birth, your address or information about your health. If you are sick or have a health condition, we might have information about that.
How long do we keep your information for?
We keep your information for as long as we are allowed to and then we delete it.
Why do we have this information?
We have this information for a few reasons:
- To help us provide health services you might need.
- To keep you and others safe.
- To follow the law
We might collect this information from you, your family and other people involved in giving you this help.
Giving your permission
Sometimes we will ask your permission to use your information.
When we ask for your permission this is called consent.
You do not have to give consent if you don’t want to – this is your choice.
If you are under 13 you may not be able to give consent on your own and we will ask your parent or guardian.
We may need to share your information with other organisations that care for you, to help keep you safe.
What decisions can you make about your information?
You can ask for your information to be corrected if it is wrong.
You can ask what information we hold about you and we can give you a copy of it.
